WireFeedWelderWASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with Star Asia USA, has announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Wire feed welders

Units: About 9,000

Importer: Star Asia USA, LLC d.b.a Titan, of Kent, Washington

Hazard: The wire welder’s torch does not have a cold contactor as erroneously stated on the packaging and instruction manual. Without this feature, the welder generates an electrical arc immediately upon contact with the welding material, posing a burn hazard to consumers.

Description: The recall involves the Vaper 90 amp flux wire feed welders with model number 41181. The wire welders are red and black. “Vaper” and “90 amp flux wire welder” can be found on the product’s operation panel. The model number can be found on the top right hand corner of the package and on the first page of the users manual.

Sold by: Pep Boys, Auto Zone and various other auto parts and tool centers nationwide from October 2006 through March 2010 for about $170.

Remedy: Consumer should immediately stop using the wire welders and contact Star Asia to obtain a corrected instructions manual and packaging or for instructions on returning the product for a full refund.

ExplosingExtinguisherExtinguishers that are bolted onto moving equipment, such as forklifts or bulldozers, should be inspected more frequently for wear and tear.

The following information was sent to JB Safety & Rescue Services by Carla Andrews, a safety compliance officer at OSHA Arizona, who received it from a global mining company in Australia.

“A fire extinguisher located on a dozer exploded, throwing the extinguisher approx 20 meters from the machine. The brackets attached to the dozer had bolts protruding enough to induce wear and fatigue cracks on the cylinder itself which resulted in a catastrophic failure in the cylinder body. When installing the brackets to the machine, care should be taken to ensure that no part of the cylinder is touching an area that may cause rubbing and wear on the cylinder body.”

Fortunately in this instance, no one was injured….or worse.  But don’t wait for catastrophe to strike your jobsite.  Check all vehicular mounted extinguishers immediately and schedule them for frequent safety wear and tear inspections.  Ms. Andrews reports that her office recently found a similar situation on an extinguisher that was mounted on a jobsite forklift.

Remember:  fire extinguishers are small pressure vessels and exploding extinguishers can be deadly.

VenomousCreaturesYou don’t need to be hiking in the wilderness to experience a venomous creature attack. This seminar will help you recognize venomous creatures, and teach you what to do to avoid being bitten or attacked.

A tragedy occurred recently involving a landscaper in Encinitas, who was killed after suffering hundreds of bee stings when his backhoe apparently disturbed a colony of bees.  In a separate incident, an 18 year old boy nearly died from a rattlesnake bite in Del Mar as well.

As many as 8,000 Americans are bitten by venomous snakes each year and dozens die, or suffer pain and disfigurement, as a result.   And that’s just snakes.  Add to that bees, wasps, spiders, scorpions, fire ants………….and you’re looking at attacks in the tens of thousands, and many more fatalities and suffering.

Review how to recognize, prevent and respond to hazardous material accidents, spills, releases, contamination and containment techniques.  We also cover any basic changes in regulatory requirements.

Genie Industries has determined that the above referenced machines were produced with the wrong floor loading information in the operator’s manual and on the decals. This information indicates that the maximum wheel load is less than the actual maximum wheel load. Incorrect floor loading information may result in the user operating the machine on a surface that is not capable of supporting the maximum wheel load.

battery_packAttention Emergency Personnel:  semi-automatic external defibrillators may falsely detect an error condition, thereby not providing therapy.

RECALL: The FDA has announced voluntary recall of Defibtech, LLC  DBP-2800 Battery Packs used in Lifeline AED and ReviveR AED (semi-automatic external defibrillators). When the AED is used with an affected battery pack, the AED may falsely detect an error condition during charging for a shock, then cancel charge and not provide therapy.

Recall affects all DBP-2800 Battery Packs shipped prior to June 4, 2007, which have been distributed globally to fire departments, EMS, health clubs, schools, and other organizations.

WHAT TO DO: Defibtech will provide customers with a free battery pack update card to address issue for all affected battery packs. The correction to the battery pack will be able to be performed at the location where the battery pack is deployed using any DDU-100 series AED and a Defibtech supplied battery pack update card.

The company has identified recommendations for the end customer to follow until the battery pack has been corrected, which allows the battery pack to remain in service. A copy of these recommendations is available at Defibtech.

CPR AED“Good Samaritans show kindness to others and their good acts should be encouraged, rather than discouraged.” – Assembly Member Mike Feuer, co-author of AB 83 (revises Good Samaritan Law)

Many of us have heard of the Good Samaritan Law which protects individuals from lawsuits while voluntarily helping others in peril.  With the passing of Assembly Bill 83, Samaritans and medical personnel alike are now covered for giving medical care AND providing related assistance – such as rescuing the person from a hazardous environment.

Cal-OSHA Title 8 section 3400 states: “In the absence of an infirmary, clinic, or hospital in near proximity to the workplace, which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid.” The Good Samaritan law now covers those who attempt to provide medical assistance to help others in a workplace emergency, without fear of liability or reprisal.
