Name of Product: THHN Electrical Wire

Units: About 1,000

Manufacturer: Cerro Wire Inc., of Crothersville, Ind.

Hazard: While the actual electrical wire has “14 gauge” printed on it, the packaging incorrectly labels the electrical wire as 12 gauge. If used as a 12 gauge wire, it can overload, posing a fire hazard to consumers.

Incidents/Injuries: None reported.

Description: This recall involves THNN electrical wire labeled on its packaging as 12 recall elec wire 2gauge solid white 100’ UPC 48243982721 and 12 gauge stranded red 50’ UPC 48243229215. The actual wire has “THHN Cerro Wire 14 gauge” printed on it. The UPC number and 12 gauge is found on the plastic wrap and on a label at the bottom of the reel.

Sold at: Home Depot & Menards stores in the following states: Colo., Iowa, Idaho, Ill., Ind., Kan., Ky., Mich., Minn., Mo., Mont., N.D., Neb., Ohio, Ore., Pa., S.D., Utah, Wash., Wis., Wyo. from December 2009 through April 2010. The 50-foot wire spools were sold for $9 and the 100-foot spools for about $16.

Remedy: Consumers should immediately stop using any switches, outlets or electrical devices using this wire and contact Cerro Wire for instructions on returning the product for a refund. Any contractor or subcontractor who used this wire should inspect their work to see that their work meets local electrical wiring code.

Hazard: The head of the sledge hammer can loosen and detach, posing a risk of impact injury to consumers.

Incidents/Injuries: None reported.

Description: This recall involves Brigade sledge hammers with a hickory wood handle. There are 11 models included in this recall, ranging in size from 2 to 20 lbs. A green and white label with the word “Brigade” and the model name is affixed to the head of the sledge hammer, and “Genuine Hickory” is printed on the handle. Model and UPC information are printed on the label.

Name of Product: Wire feed welders

Units: About 9,000

Importer: Star Asia USA, LLC d.b.a Titan, of Kent, Washington

Hazard: The wire welder’s torch does not have a cold contactor as erroneously stated on the packaging and instruction manual. Without this feature, the welder generates an electrical arc immediately upon contact with the welding material, posing a burn hazard to consumers.

Description: The recall involves the Vaper 90 amp flux wire feed welders with model number 41181. The wire welders are red and black. “Vaper” and “90 amp flux wire welder” can be found on the product’s operation panel. The model number can be found on the top right hand corner of the package and on the first page of the users manual.

Sold by: Pep Boys, Auto Zone and various other auto parts and tool centers nationwide from October 2006 through March 2010 for about $170.

Remedy: Consumer should immediately stop using the wire welders and contact Star Asia to obtain a corrected instructions manual and packaging or for instructions on returning the product for a full refund.

Genie alertIf you own or operate a Genie Z-80 Boom Lift, especially a model within the following range of serial numbers  – Z80-101 to Z8009-2448 – contact your Genie dealer for more information.

Genie Industries has determined that the above referenced machines were produced with the wrong floor loading information in the operator’s manual and on the decals. This information indicates that the maximum wheel load is less than the actual maximum wheel load. Incorrect floor loading information may result in the user operating the machine on a surface that is not capable of supporting the maximum wheel load.

If you are the owner or operator of an affected machine and are not an authorized Genie dealer, please contact your nearest Genie dealer for assistance.

Genie and ANSI requires that the seller of a Genie machine report to Genie the model and serial number of each machine sold, as well as the name, address, and telephone number of the new owner, within 60 days of the sale. Nailers may cause serious injury hazard from faulty feeder.

WASHINGTON, DC – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Coil Nailers

Units: About 50,000 in the United States and about 15,000 in Canada

Importer: Hitachi Koki U.S.A., Ltd., of Norcross, Ga.

Manufacturer: Hitachi Koki Co. Ltd., of Japan

Hazard: The nailers could have a faulty feeder that can allow nails to be ejected sideways, posing a serious injury hazard to the user or bystanders.

Incidents/Injuries: The firm has received 37 reports of nails being ejected sideways, including 15 reports of injuries. The injuries were primarily in the eye region, including five reports of partial blindness.

Description: The coil nailers are used to project nails into drywall, wood or other materials. The model number is NV83A2 and can be found on the body of the product. Only those units manufactured between October 2002 and September 2005 are included in this recall. The manufacturing date can be identified by the serial number engraved at the end of the handle, the first digit representing the month (1 for January, 2 for February, 3 for March, 4 for April, 5 for May, 6 for June, 7 for July, 8 for August, 9 for September, O for October, N for November and D for December) and the second digit representing the year (2 for 2002, 3 for 2003, 4 for 2004 and 5 for 2005).

Sold at: Lowe’s, Home Depot, other home improvement and building supply stores and online at Amazon.com nationwide from November 2002 through March 2006 for between $350 and $400.

Manufactured in: Japan

Remedy: Consumers should immediately stop using the recalled coil nailer and contact Hitachi Koki U.S.A., Ltd. for a free repair.

Fluke RecallMarch 8, 2010, San Diego, CA –Thanks to Bergelectric Corp. for providing the following  safety recall on an instrument that is critical to electrical safety.

In cooperation with the U.S. Consumer Product Safety Commission, Fluke is voluntarily recalling its Fluke 1AC-I VoltAlert TM voltage tester, a product that was first released for sale on September 29, 2009. The affected model number is listed in the table below. If you own a Fluke 1AC-I (1AC-A1-I), stop using it and send it back to Fluke for a replacement unit.

If you are not the primary user of the 1AC-I please pass this notice along to the appropriate people within your organization.

Problem: The testers can fail to give an indication of live voltage, resulting in the operator falsely believing the electrical power is off, posing a risk of serious injury or death from electrical shock or thermal burns.

Name of Product: Outdoor Lighting Fixtures

Units: About 900

Manufacturer/Distributor: American Electric Lighting, a division of Acuity Brands Lighting Inc., of Conyers, Ga.

Hazard: Improper wiring in the light fixtures poses a shock hazard to consumers.

Incidents/Injuries: None reported.

Description: This recall involves model AVL outdoor lighting fixtures sold under the American Electric Lighting brand name. The light fixtures are intended for use in parking lots, roadways, commercial environments and office communities. “AVL” is printed on the reflector of each light fixture.
Sold by: Electrical distributors and electrical sales representatives nationwide from September 2008 through September 2009 for between $250 and $800.

Manufactured in: Mexico

Remedy: Consumers should immediately disconnect power to the fixtures and contact American Electric Lighting to schedule a free inspection and repair.

Only 8000 model – not other N95 versions of other 800o Series of respirators – are affected.

An important note on state respirator recall Cal-OSHA Reporter’s Jan. 4 flash report about the recall of 3M 8000 respirators made reference to the “8000 series.”

That term has triggered a deluge of questions to Cal/OSHA about whether other 3M respirators are affected by the recall, which was ordered when health-care employers reported an unacceptably high failure rate during fit tests.

For instance, Craig Brown, senior industrial hygienist for the VHA Center for Engineering & Occupational Safety and Health in Pittsburgh, Pa., notes that there are several sub-series of the 3M 8000 N95 respirators with different face molds, such as 8210, 8211, 8511, 8612, 8670 and more.

The only respirators that have been recalled are the 8000s, says DOSH Senior Safety Engineer Deborah Gold. “Other N95 respirators are not affected by the alert,” she says.

V Guard Hard Hats Alert 2MSA will replace any V-Gard hats manufactured in 3/2008 that exhibit cracks through normal (non-impacted) wear

Safety equipment manufacturer, MSA, reports that “a small number of V-Gard Caps manufactured in March 2008 may be susceptible to premature cracking, which we have found to be attributable to minor variances in our colorant.”

If a V-Gard Cap exhibits this type of crack, it is readily noticeable at the top of the shell and easily identified through routine, daily inspection.

MSA has informed us their testing indicates V-Gard Caps exhibiting this type of crack continue to meet all ANSI Z89.1-2003 and CSA Z94.1-2005 impact and penetration test requirements. However, a crack of this type would prevent the cap from passing the electrical resistance test.

V Guard Hard Hats alertCaps that do not exhibit a crack and meet all other inspection requirements are certainly suitable for use and should remain in service.

Please remember to inspect your V-Gard Caps before and after each use. MSA will replace any V-Gard Caps manufactured in March 2008 that exhibit this type of crack without being impacted. Voluntary Recall by Manufacturer Due to Potential Fall Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission announced a voluntary recall; consumers should stop using the harness and contact Gorilla Inc. of Flushing, Michigan for a refund.  This harness is primarily used as a fall restraint.  The webbing of the waist belt on the safety harness is not routed through the lineman’s loop located on the front of the harness near waist level. Since the loops are not properly anchored to the harness webbing but are attached only through stitching not intended to restrain a user during a fall, they can pull away from the harness when force is applied, leaving the user unrestrained.

The name EXO-Tech is located on the right front shoulder strap and the name Gorilla is located on the left front should strap both in white lettering. Similar to a shirt tag, there is a white manufacturing label on the inside of the back of the harness with the model number 45111 and manufacturing dates, 4/22/2009 or 6/26/2009. These are the only harnesses recalled.

Harnesses were sold through various sporting goods stores nationwide and on-line from May 2009 to August 2009.
